I would like to take out a day permit for Great Westwood Equestrian Park. I have read and understand the GWEP Rules & Conditions - http://www.greatwestwood.co.uk/page12.htm - and I agree to observe them. I confirm that I have adequate Personal Injury and Third Party Liability Insurance for my horse/pony and will indemnify the owners of Great Westwood Equestrian Park against all claims, for which I could be held responsible, arising from my use of the tracks, gallops and jumps, or from any breach of my obligations contained in this application form and the GWEP Rules & Conditions. This form will apply for any future permits taken out by myself.
